The judge failed to disclose her relationship to an attorney while presiding over a case in which the attorney represented a party. The judge further appointed the attorney as an ad litem in several proceeding in her court, awarded the attorney fees, and failed to voluntarily recuse herself from those cases and/or disclose the relationship with the attorney to the parties in those cases.
